Turning 18: Health & Social Care Funding

Step by step

  1. Step 1

    Confirm current funding: children's continuing care / CHC / LAC status.

  2. Step 2

    Ask for a Preparing for Adulthood (PfA) review 6–12 months before 18.

  3. Step 3

    Request adult CHC checklist if health needs are significant.

  4. Step 4

    If eligible for CHC at 18, agree the Personal Health Budget plan.

  5. Step 5

    If not CHC, ask Adult Social Care for a Care Act assessment.

  6. Step 6

    If there's a primary mental health need, check Section 117 aftercare duties.

  7. Step 7

    Align funding with the EHCP and any Direct Payments/Personal Budgets.
